Solicitud Para La Tarjeta Estudiante De La Biblioteca

Student Library Card Application

Getting a library card is important. It is the first membership your child will keep for their entire life. Research shows that young people who read have:

Student Birthdate
Grade *
Notice Preference *
Notice preference is how you would be notified if a book placed on hold had come in for you or if an item checked out was due to be returned.
Help your child choose 4 numbers they can remember. The PIN is the password that goes with their library card number.
Agreement *
I understand that I am responsible for all use made of my library card and I agree to abide by library rules. This card may be used at all public libraries in Clackamas County. Policies and services vary between libraries. Information about a member's record cannot be given to anyone but the member. The state of Oregon takes a strong stand on an individual’s right to privacy. Children in Oregon have full privileges as library members. As a parent, to access your child’s library record you will need to have their library card or card number in hand. We encourage you to talk to your children about your family’s values and what appropriate use of the library means to you.
