To apply for enrollment in the City of Sandy FOG Grant Program, please fill out the entire application:

Make sure you fill out every field in the application. All fields are required.
Once you have completed the application, click on "Submit".
You will receive a confirmation message declaring that the City of Sandy has received your application.
Once we've processed your application, city staff will contact you about next steps.
